Parents are responsible for maintaining current contact information and updating the school with any new information on student’s allergies or medical conditions. All current medical diagnosis/conditions and activity restrictions require documentation from the student’s licensed health care provider each school year.
Reminder DO NOT SEND MEDICATIONS TO SCHOOL WITH YOUR STUDENT – must be signed into the health room by a parent/guardian including OTC medications
All medications including OTC must have Dr. order . Medications that are expired, not in original packaging or missing required documentation will not be accepted. Please reach out to the school nurse directly with any questions.
Health Screening Consent Form
Health screenings are offered by the health department. Please note all health screenings are voluntary and only performed with written parent consent. Not all of the screenings listed on district form are offered at NCCS. Screenings are provided by the health department’s guidelines for recommended grade levels.
